This one's tough. I don't have any idea. Trying to trace back the roots, I'm pretty sure it's not #3 or #4 and I agree that #5 has to be made up.
So #1 or #2. Eenie Meanie...

You're right - the word "abnormal" may be redundant. Maybe my selection criterea was faulty. |

Well, fear of dental hygienists wasn't an option, so I'm going for #1 as my mother-in-law was a dental hygienist, I think.
